Medicaid Plan Changes

On January 1st, the state moved Medicaid members at random to different plans and providers.

 

You must call your plan to confirm your child is assigned to one of the providers shown below:

• Ashley Viviano, Nurse Practitioner

 

The clinic will not be able to see your child until he/she is assigned to one of the providers above.
