New Patient Information
We understand choosing a pediatric healthcare provider can be a tough decision so we would like to make the first visit as easy as possible.
Expecting parents
To help you get more acquainted with our office, please schedule a visit for a tour and to meet our providers.
While in the hospital, notify the nurse and on-call doctor seeing your child that you will be choosing Progressive Children's Clinic as your primary care provider. We typically do not see your child while in the hospital so we recommend the first visit be with in 3-5 days after being discharged from the hospital. Please make sure to bring your baby's hospital discharge paperwork too.
What to expect in our office
Upon entering, please sign in with the front desk receptionist. You will be given appropriate documents to complete. You will need to complete the documents before being seeing the provider. Please make sure to bring:
-
List of current medications
-
Insurance cards
-
Identification
-
Completed new patient registration. Please only complete the new patient registration once you have scheduled an appointment. (Click here to complete NEW PATIENT REGISTRATION online). If you did not complete the registration packet online, please arrive 20 minutes early to complete the necessary paperwork.
Please have a seat in our waiting room. We have a waiting area for our newborn through 2 month old children. A well waiting area for children here for a well-child check up and a waiting area for ill children who may be running fever.
Next you will be called to the triage area. Your child will have his or her vital signs and measurements taken. You will then be taken to your exam room and the provider will be in to see you.
We do our very best to see you as close to your appointment time as possible and we strive to give each child the personal attention and time needed during his visit.
During your visit, questions are encouraged and welcomed!
